Driver

Job Summary:

Under the supervision of the Transportation Coordinator is responsible for the safe and timely transportation of PACE participants.  Including transport of participants, meals, durable medical equipment, pharmacy, and any other transportation as deemed necessary.  Responsible for overall cleanliness and safety of the vehicle assigned on a daily basis.  Responsible for communicating any safety issue to the Transportation Coordinator.

  • CA driver’s license
  • Safe driving record
Skills and Knowledge:
  • Able to read and understand maps and routing information. 
  • Able to maintain confidentiality at all times. 
  • Skilled in establishing and maintaining effective relationships with participants, caregivers, co-workers, and business associates. 
  • Experienced in transporting individuals with special mobility needs based on experience or demonstrated post training. 
  • Must be flexible and able to function in stressful situations 
  • Knowledge of safety requirements pertaining to the transportation industry. 
  • Requires individual who can coordinate multiple tasks simultaneously 
  • Proficient in identifying and recommending problem resolution. 
  • Ability to work independently with minimum supervision 
  • Requires corrected vision and hearing to normal range, with or without reasonable accommodation
Experience:

Minimum of 1 year transporting the frail and elderly or driver training specific to PACE.

Medical Clearance:

Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

Competency:

Position specific competencies for the Nurse Practitioner will be met prior to assuming participant care.

Physical Demands and Working Conditions:

The physical demands and work environment characteristics described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Work Environment:

Employee must be able to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the PACE participants’ care. The work setting is in an Adult Day Health Center and primary care clinic environment with moderate noise levels and controlled temperatures.

Physical Requirements:

Requires corrected vision and hearing to normal range. Must be able to communicate with all staff, vendors, and clients. Requires manual and finger dexterity and eye-hand coordination; the ability to use department equipment. Requires lifting/carrying up to 65lbs.and pushing or pulling 150lbs.using appropriate body mechanics and equipment, with reasonable accommodation if needed.

Visual, Hearing and Communication Requirements:

Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Ability to communicate by way of the telephone with participants, customers, vendors and staff.

Pressure Factor:

Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuating indoor temperatures. Moderate pressure to meet scheduled deadlines. Potential for exposure to verbal aggression by client, vendors, and staff.

Environmental Conditions:

May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases and any other conditions common in a healthcare environment. Subject to unpleasant odors.

Duties and Responsibilities:

Responsible for PACE passengers during transport, providing care in a manner which ensures participant satisfaction. 

Follows assigned schedule, makes adjustments to meet unanticipated changes while considering the needs of the participant and requirements of the PACE Center. Communicates these changes to Transportation Coordinator or designee. 

Operates equipment safely and in compliance with vehicle licensing regulations. 

Maintains and completes daily required records, such as Pre and Post Trip Inspections, gasoline purchases, etc. 

Immediately reports to Transportation Coordinator any vehicle or cell phone operating problems, malfunction, or incidents. 

Transports participants to and from medical/specialist appointments and activity outings as requested by the interdisciplinary team per established transportation request process. 

Transports meals, durable medical equipment, pharmacy, home care supplies, or other items deemed necessary. 

Provides physical assistance to each participant when needed as s/he boards and disembarks from the program’s vehicles. Provides door-through-door assistance. 

Inspects vehicle at the end of each day and maintains the vehicle in a neat and clean condition. 

Ensures vehicle equipment is maintained and operating within manufacturer’s recommendations, replacing supplies as needed. 

Functions as a member of the interdisciplinary team when requested. 

Communicates with Transportation Coordinator or designee daily to be updated on participant conditions/changes, plan of care changes, or interdisciplinary team requests. 

Follows policies and procedures and OSHA safety guidelines. 

Maintains the confidentiality of all company procedures, results, and information about participants, clients, or families. 

Handles potentially infectious fluids (e.g., blood, urine, emesis) with appropriate biohazard precautions and practices Standard Precautions. 

Participates in and supports Quality Improvement initiatives. 

Participates in continuing education classes and any required staff and training meetings. 

Maintains any required certifications or licenses. 

Performs other duties as required or requested.
