Center Manager

  • Supervised by: Executive Director
  • Supervises: Clinic Supervisor, Day Center Supervisor, Physical Therapist, Occupational Therapist, Speech Therapist, Recreational Coordinator, Social Worker, Dietitian, Receptionist

Job Summary:

Under the supervision of the Executive Director, oversees and manages the daily operations of [ AHHC ] including operations of the day center, clinic, social work, rehabilitation, recreation, spiritual care and dietary.  This includes ensuring that appropriate personnel are trained and perform their functions within the organization, operational services standards and budgetary criteria are met, and operations are in compliance with all governing regulations.  Assumes the Executive Director’s responsibilities in her/his absence.

Qualifications and Requirements:

Education/Training/Certifications:
  • Bachelor’s Degree in business administration, or a health or human services field, or Registered Nursing degree/diploma required.  Professional licensure and Master’s degree preferred.
  • Valid [State] Drivers License.
Experience:
  • A minimum of five (5) years of health care experience, and three (3) years of management experience, preferably in a geriatric care setting required.
  • A minimum of one year’s experience working with the frail or elderly population is required.   
  • Either training or related experience in direction of a health care program.
  • Knowledge of the physical, mental, and social needs of the elderly.
Skills and Knowledge:
  • Working knowledge of the PACE Regulations and all other Regulatory requirements.
  • Working knowledge of the state’s healthcare system, local health care and aging networks.
  • Demonstrated effective leadership and decision-making skills.
  • Effective supervisory skills.
  • Ability to foster collaborative working relationships.
  • Strong group facilitation skills.
  • Working knowledge of physical, mental, and social needs of the frail elderly and their families.
  • Working knowledge of quality improvement and care management systems.
  • Ability to apply creative problem-solving skills to complex issues.
  • Ability to effectively and efficiently plan, prioritize and follow-up on delegated responsibilities.
  • Grasp of the knowledge base from which various disciplines assess the needs of the elderly population.
  • Effective written and oral communication skills.
  • Working knowledge of spreadsheets, databases, word processing, and Electronic Health Records.
Medical Clearance:

Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

Physical Demands and Working Conditions:

The physical demands and work environment characteristics described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. In compliance with ADA requirements, reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.

Work Environment:

Employee must be able to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the PACE participants’ care. The work setting is in an Adult Day Health Center and primary care clinic environment with moderate noise levels and controlled temperatures.

Physical Requirements:

Requires manual and finger dexterity and eye-hand coordination; the ability to use department equipment; and the ability to lift/carry up to 30 pounds using appropriate body mechanics with reasonable accommodation if needed.

Visual, Hearing and Communication Requirements:

Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Ability to communicate by way of the telephone with participants, customers, vendors and staff.

Pressure Factor:

Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuating indoor temperatures. Moderate pressure to meet scheduled deadlines. Potential for exposure to verbal aggression by client, vendors, and staff.

Environmental Conditions:

May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases and any other conditions common in a healthcare environment. Subject to unpleasant odors.

Duties and Responsibilities:

Oversee the daily coordination and the implementation of the PACE Model of Care.

Continuously evaluate the participants’ changing needs and makes necessary program adjustments.

Assist in providing in-service education for the PACE staff.

Direct, supervise, and evaluate the performance of the Clinic Supervisor, Day Center Supervisor, Physical Therapist, Occupational Therapist, Speech Therapist, Recreational Coordinator, Social Worker, Dietitian, Receptionist.

Ensure continuity of operations

Assume the Executive Director’s responsibilities in her/his absence.

Ensure that the job responsibilities, authorities and accountability of all direct reports are defined and understood.

In conjunction with the Executive Director, responsible for maintaining compliance with all state and federal program requirements, obtaining and maintaining certification under Centers for Medicare and Medicaid Services (CMS) as well as any other state or federal law or rules.

Assist in the development and implementation of the organization’s PACE Strategic Plan.

Assist in program development and preparation of [ AHHC ]’s short-range and long-term plans, budgets and related information systems based on broad organization goals and growth objectives.

Participate in all interdisciplinary team meetings and serves as facilitator.

Coordinate the interdisciplinary team’s care management process to ensure best decisions are made regarding the participant’s changing condition.

Oversee and direct the coordination of home health, rehabilitative, long-term care and medical/specialty services according to participant care plans.

Assist Executive Director in development of annual operational and capital budgets for departments of accountability and managing services to meet financial benchmarks.

Assist Human Resources Generalist in development and implementation of PACE operational policies, procedures, protocols and regulations for direct participant care and support services.

Monitor operations to ensure consistent compliance with established quality assurance standards.

Assist Executive Director with evaluation of [ AHHC ] performance against national results and established service standards, budget and other financial criteria.

Assist in development and implementation of performance improvement plans based on Quality Assessment findings. 

Recommend changes in service delivery and staffing as required due to the development and growth of the organization.

Participate regularly in all quality assurance/quality improvement committees as part of the overall quality improvement program.

In conjunction with the Executive Director and the Quality Assurance Director, maintains the stability and reputation of [ AHHC ] by ensuring that all activities and operations are performed in compliance with local, state, and federal contracts, regulations, laws, accreditation, protocol, licensing and certification requirements governing  operations.

Establishes positive working relationships with health and social service providers.

Monitor operational compliance with Occupational Safety and Health Administration (OSHA) regulations and agency policies and procedures.

Protect privacy and maintains confidentiality of all company procedures, results, and information about, employees, participants, clients or families.

Participate in continuing education classes and any required staff and training programs.

Maintain professional affiliations and any required certifications.

Provide administrative on-call coverage on a rotation basis.

Perform related duties as required.
