Transportation Coordinator

  • Supervised by: Executive Director
  • Supervises: Drivers

Job Summary:

Under the supervision of the Executive Director, is responsible for the PACE Transportation program, including: transportation of participants, meals, durable medical equipment and medications; transportation scheduling; oversight, hiring, training and supervision of all drivers and dispatchers and any other transportation as deemed necessary. Oversees vehicle repairs and maintenance. Identifies quality and cost effective vendor for same. Responsible for meeting departmental budget goals.

Certifications:
  • Valid drivers license
Experience:
  • 2 years of administrative/supervisory experience, in a transportation setting.
  • 5 years of experience in the transportation industry preferred.
  • Minimum of 1 year working with the frail or elderly population or training and tested competency provided by [Asian Heritage Healthcare].

Work Environment:

Employee must be able to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the PACE participants’ care. The work setting is in an Adult Day Health Center and primary care clinic environment with moderate noise levels and controlled temperatures.

Physical Requirements:

Requires corrected vision and hearing to normal range. Must be able to communicate with all staff, vendors, and clients. Requires manual and finger dexterity and eye-hand coordination; the ability to use department equipment. Requires lifting/carrying up to 65lbs.and pushing or pulling 150lbs.using appropriate body mechanics and equipment, with reasonable accommodation if needed.

Visual, Hearing and Communication Requirements:

Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Ability to communicate by way of the telephone with participants, customers, vendors and staff.

Pressure Factor:

Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuating indoor temperatures. Moderate pressure to meet scheduled deadlines. Potential for exposure to verbal aggression by client, vendors, and staff.

Environmental Conditions:

May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases and any other conditions common in a healthcare environment. Subject to unpleasant odors.

Duties and Responsibilities:

Supervise the transportation departmental staff. Responsible for departmental hiring, orientation, competency, and training procedures. 

Participate in the interdisciplinary team meetings, communicating participant changes, collaborating on care planning decisions and coordination of 24 hour care delivery.

Communicate interdisciplinary team decisions, participant changes, care plan changes, and updated information to transportation staff on a daily basis. 

Ensure responsibility for running and maintaining transportation scheduling software program. 

Ensure effective daily communication between PACE Center staff, transportation drivers, and dispatch. 

Ensure the scheduling, coordination, communication of the driver route assignments of [Asian Heritage Healthcare] and assure the daily work schedule is communicated. 

Ensure the scheduling of transportation for all participants outside appointments. 

Monitor use of driver/dispatch overtime, ensure adequate staffing and improve efficiencies. 

Coordinate and schedule the daily delivery of home delivered meals, durable medical equipment (DME), supplies and miscellaneous items as requested by the IDT team. 

Manage and update the participant database. 

Collaborate with the Executive Director in the annual and capital budget development and maintain the departmental budget within its allowed and reasonable objectives as well as meets established budgetary guidelines.

Oversee vehicle maintenance program and obtain estimates and authorize vehicle repairs per departmental policies. 

Research new vehicle options and assist in the ordering and purchasing of new vehicles. 

Ensure that vehicles have up-to-date Department of Motor Vehicle registration forms, license plates and emission stickers. 

Ensure cleanliness and proper functioning equipment as well as safe driving conditions for each vehicle. 

Ensure that all departmental staff attend and successfully execute required annual driver training courses and participate in mandatory in-service training. 

Set clear expectations and assign appropriate authority to staff; hold staff accountable for their scope of work and compliance to company policies and procedures. 

Follows all Policies and Procedures and OSHA safety guidelines. 

Maintain the confidentiality of all company procedures, and information about participants, clients or families. 

Handle potentially infectious specimens with appropriate biohazard precautions, and practice standard precautions. 

Participate in and supports Quality Improvement initiatives. 

Participate in continuing education classes and any required staff and training meetings. Maintain professional affiliations and any required certifications. 

Perform other duties as required or requested.
